Membership Application
- All original documents must be submitted & verified in support of your application.
- Copy of one valid form of ID (National ID, Passport, or Driver’s Permit).
- Recent payslip or job letter (≤3 months old).
- Recent utility bill, lease, or rental agreement (if unavailable: owner letter + ID required).
- Self-employed: statement of income, BIR number, VAT certificate (if applicable).
- Eligible for CUNA Indemnity Plan upon acceptance.
- Fees: $5 Entrance Fee | $100 Share Deposits.
- All original documents must be submitted & verified in support of your application.
- Youth Member’s Documents: Birth certificate, Passport/ID (if available).
- Adult Opening/Servicing Documents: Valid ID, payslip/job letter (≤3 months), utility bill (or owner letter + ID), self-employed docs.
- Fees: $5 Entrance Fee | $50 Share Deposits.
